Job Summary
Assess assigned case loads of behavioral health patients and families to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary reports. Plan, coordinate, implement, and evaluate treatment plans while making initial and follow-up visits to re-evaluate nursing needs. Counsel patients, families, and facilities on the interdisciplinary plan of care and teach behavioral health philosophy and symptom management. Initiate and supervise behavioral health aide and LPN care delivery, documenting observance per agency policy. Communicate significant findings, unsafe conditions, and changes in health status to the Clinical Supervisor, physician, and facility. Provide support at time of death and perform bereavement assessments. Requires current unrestricted state RN license, 1+ years of clinical experience, and ability to travel 50% within the Norwich, New London, and Groton coverage area. Preferred qualifications include an Associates or Bachelor's degree in Nursing and previous home health or hospice experience.
Required Qualifications
- High School Diploma or GED
- Current, unrestricted state RN licensee
- 1+ years of experience in a clinical care setting as a nurse
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
Desired Qualifications
- Associates Degree or Bachelors Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with Homecare HomeBase is preferred
