Registered Nurse Psych Visits Part Time AM

Assess assigned case loads of behavioral health patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary reports. Plan, coordinate, implement, and evaluate treatment plans while making initial evaluations and follow-up visits based on patient, family, and facility requirements. Counsel patients, families, and facilities on the interdisciplinary plan of care and teach behavioral health philosophy and symptom management to staff in alternate settings. Initiate and supervise behavioral health aide and LPN care delivery, documenting observance per agency policy. Communicate significant findings, problems, and unsafe facility conditions to the clinical supervisor, physician, and other personnel. Provide support at time of death and perform bereavement assessments. Work part-time Monday through Friday 7a-1p in the Hartford and Glastonbury areas.
