Job Summary
Registered Nurse Behavioral Health visits in Granby and Ludlow, MA. Provides 1:1 patient care in the home health setting with flexible days and a focus on keeping clients in their homes. Responsibilities include assessing caseloads and determining physical, psychosocial, and environmental needs; planning, coordinating, implementing, and evaluating the plan of care; communicating findings to the interdisciplinary team; teaching and supporting patients, families, and facility staff in behavioral health care and symptom management; initiating, supervising, and documenting delivery of care per agency policy; and traveling within the branch coverage area. Requires an RN license, high school diploma or GED, 1+ year clinical experience, and willingness/ability to travel up to 50% with a valid driver's license and auto insurance. Base salary range: $85,000-$90,000 annually; comprehensive benefits and tuition/education opportunities included.
Required Qualifications
- High School Diploma or GED
- Current, unrestricted state RN license
- 1+ years of experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance
