Job Summary
Registered Nurse Behavioral Health home visits in Chicopee, MA. Assess and manage a caseload of behavioral health patients in their homes, identify physical, psychosocial, and environmental needs, and develop and implement an interdisciplinary plan of care. Provide 1:1 patient care, conduct initial evaluations and ongoing re-evaluations, and adjust plans as needed. Communicate findings to the Clinical Supervisor, physician, and care team; educate patients, families, and facility staff on behavioral health care, pain and symptom management, and care-plan adherence. Travel within the coverage area to deliver care, with flexible days and the potential for 50% travel. Requires RN licensure, 1+ year clinical nursing experience, ability to lift up to 50 pounds, and a dependable vehicle with valid license and auto insurance. Strong teamwork, documentation, and comfort working in home-based settings are essential; opportunities for tuition reimbursement and ongoing education are available. This role emphasizes autonomy in 1:1 patient care, collaboration with an interdisciplinary team, and delivering high-quality behavioral health services in patients’ home environments.
Required Qualifications
- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current, unrestricted state RN license
- 1+ years of experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Associates Degree or Bachelors Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with Homecare HomeBase is preferred
