Registered Nurse Psych Visits FT

Registered Nurse Behavioral Health Visits for home-based care in Westfield, MA. Responsible for assessing a caseload of behavioral health patients and families to identify physical, psychosocial, and environmental needs; planning, coordinating, implementing, and evaluating the plan of care; conducting initial evaluations and periodic re-evaluations; performing follow-up visits; communicating findings to the Clinical Supervisor, physician, and care team; updating the plan of care as needed; instructing and involving patients, families, and facility staff in the interdisciplinary plan of care; educating staff on behavioral health principles and pain management; supervising delivery of care by aides and LPNs according to policy; flagging and addressing unusual or potentially problematic facility issues; and supporting bereavement assessments. Requires a High School Diploma/GED, RN licensure, and willingness to travel within the coverage area.
