Job Summary
Assess assigned case loads of behavioral health patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary reports. Assume primary responsibility for the plan of treatment by assessing, coordinating, implementing, and evaluating care while making initial evaluations and follow-up visits. Counsel patients, families, and facilities on the interdisciplinary plan of care and teach behavioral health philosophy and pain management to staff in alternate settings. Initiate and supervise behavioral health aide and LPN delivery of care per agency policy, communicating significant findings and unsafe facility conditions to supervisors and physicians. Provide support at time of death and perform bereavement assessments. Full-time days with a 50% travel requirement and a $5,000 sign-on bonus.
Required Qualifications
- High School Diploma or GED
- Current, unrestricted state RN licensee
- 1+ years of experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must be able and willing to travel 50%
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
Desired Qualifications
- Associates Degree or Bachelors Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with Homecare HomeBase is preferred
