Job Summary
Assess assigned case loads of behavioral health patients and families to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary reports. Assume primary responsibility for the caseload by assessing, planning, coordinating, implementing, and evaluating the plan of treatment while making initial and follow-up visits. Counsel patients, facilities, and families in following the Interdisciplinary Plan of Care and teach behavioral health philosophy and symptom management to staff in alternate care settings. Initiate, supervise, and document the observance of behavioral health aide and LPN delivery of care per agency policy. Communicate significant findings, problems, and changes in health conditions or unsafe facility conditions to the Clinical Supervisor, physician, and other personnel. Provide appropriate support at time of death and perform bereavement assessments.
Required Qualifications
- High School Diploma or GED
- Current, unrestricted state RN licensee
- 1+ years of experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must be able and willing to travel 50%
Desired Qualifications
- Associates Degree or Bachelors Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with Homecare HomeBase is preferred
