Job Summary
Assess assigned case loads of behavioral health patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary reports. Assume primary responsibility for the caseload by assessing, planning, coordinating, implementing, and evaluating the treatment plan. Make initial evaluations and re-evaluate nursing needs during each visit, then make follow-up visits based on patient, family, and facility requirements. Counsel, instruct, and include patients, facilities, and families in following the Interdisciplinary Plan of Care. Teach behavioral health philosophy and pain management to staff in alternate care settings. Initiate, supervise, and document observance of aide and LPN delivery of care per agency policy. Inform supervisors of unusual or problematic nursing facility issues and provide support at time of death.
Required Qualifications
- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- 1+ years of experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must be able and willing to travel 50%
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
Desired Qualifications
- Associates Degree or Bachelors Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with Homecare HomeBase is preferred
