Registered Nurse Psych Visits FT 5K sign on

Assess assigned case loads of behavioral health patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary reports. Plan, coordinate, implement, and evaluate treatment plans while making initial and follow-up visits according to patient and facility requirements. Counsel patients, families, and facilities on the interdisciplinary plan of care, teach behavioral health philosophy and symptom management, and supervise aide and LPN delivery of care. Communicate significant findings, unsafe conditions, and changes in health status to the Clinical Supervisor, physician, and other personnel. Provide bereavement assessment and support at time of death. Requires current unrestricted RN license, 1+ years of clinical experience, valid driver's license, and ability to travel 50% within Chicopee area. Full-time role with $85,000–$90,000 base salary and 5K sign-on bonus.
