Job Summary
Provides home health nursing care, develops and initiates the plan of care, coordinates with physicians and caregivers, and oversees implementation of the plan of care in patients' homes and communities. Performs patient and caregiver education, prepares clinical notes, communicates with physicians and other health care practitioners, identifies discharge planning needs, and participates in on-call duties. Supervises home health aides and other nursing staff as directed; ensures availability of equipment and supplies; conducts ongoing assessment and documentation to support individualized care plans and quality improvement efforts.
Required Qualifications
- Must have graduated from an approved school of nursing and be licensed in the state where practicing.
- Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred.
- Maintains a current CPR certification.
- The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.
