Registered Nurse PRN

Monitor chronic health conditions, admit residents, and coordinate discharge planning while ensuring physician orders are in place. Conduct comprehensive, quarterly, and change-of-condition assessments using MDS forms to contribute to individualized care plans. Provide compassionate nursing care for residents with cognitive decline, memory loss, or trauma history, while assessing and addressing pain. Administer medications per orders, monitor weight and intake, and provide preventative skin care and wound treatments. Assist with activities of daily living, reposition bed-bound residents, and collect laboratory specimens. Collaborate with hospice services for end-of-life care, promote falls prevention, and oversee CNAs as directed.
