Job Summary
Conduct patient home visits in North Philadelphia, collaborating with physicians, therapists, and social workers to deliver comprehensive care. Assess, plan, direct, and evaluate each patient's care plan while managing emergency situations promptly. Complete medication administration and treatment needs, then prepare and maintain accurate clinical and progress notes per regulations and company policy. This Registered Nurse, PRN role supports our mission to provide exceptional home health and hospice services to patients in their homes.
Required Qualifications
- Active RN state license
- Valid drivers' license
- Two or more years of applicable nursing experience
Desired Qualifications
- Home Health experience (preferred)
