Job Summary
Respond to clinical referral information with care, assessing client eligibility for specialized hospice services and guiding them through every step of their journey. Partner with the Clinical Director to coordinate care plans, ensuring patient comfort and quality of life remain paramount while delivering hands-on nursing skills rooted in proven techniques. Empower patients and families through education, encouraging active participation in personalized care plans, and share the responsibility of call duties to support those under our care. Required Qualifications
- Hold current unencumbered license as a Registered Nurse
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Must be computer proficient in typing and various programs, including background in EMR
- Possess and maintains current CPR certification if required by state
- Current driver's license
- Vehicle insurance
- Access to dependable transportation
