Job Summary
Administer nursing care for clients in their place of residence, coordinate care with the interdisciplinary team, clients, and families, and complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs. Regularly re-evaluate client needs, develop and revise the Plan of Care, initiate preventative procedures, and plan for discharge while applying infection control and universal precautions. Provide effective communication to all stakeholders, monitor cases for third-party payer compliance, and supervise, teach, and provide clinical direction to other nursing personnel. Perform on-call responsibilities as assigned, maintain mandatory continuing education, and work a flexible schedule with local travel.
Required Qualifications
- One (1) year experience as an RN in a clinical care setting
- Completion of an accredited Registered Nursing Program
- Current driver's license in good standing
- Current State professional license as an RN in any/all applicable states
- Current CPR
- Must read, write and comprehend English
- If required to make visits, Hepatitis profile
- Prolonged standing/walking
- The ability to lift/transfer up to 50 lbs
- Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions
- Ability to travel locally and work a flexible schedule
- Reliable transportation and auto liability insurance
Desired Qualifications
- Preferred Home health experience
- Preferred, BSN
