Job Summary
Registered Nurse in Primary Care Solutions provides healthcare of the highest quality to America’s Veterans as part of a Patient Aligned Care Team (PACT) with an active role in developing and implementing a patient care plan. Full-time onsite role with office hours Mon-Fri 8:00am–4:30pm. Responsibilities include assisting practitioners with patient care and nursing skills, triaging urgent/emergent same-day patients, coordinating patient care, participating in daily team huddles, managing patient communications, educating patients and families on self-management and prevention, monitoring follow-up, and promoting clinical goals. Requires 1 year of Primary Care or Chronic Disease Management experience, active nursing license, BLS, and EMR proficiency.
Required Qualifications
- 1 year of Primary Care and/or Chronic Disease Management experience
- Current, active, and unrestricted state Nursing License
- Current BLS for providers from the American Heart Association (to be renewed annually)
- Strong computer skills, including EMR experience
