Job Summary
Provide hands-on nursing care across hospital, clinic, and long-term care settings; complete comprehensive patient assessments, monitor vital signs, administer medications and treatments, document accurately in the Electronic Medical Record, and collaborate with physicians, therapists, case managers, and support staff to develop and implement individualized care plans. Educate patients and families about diagnoses, medications, follow-up care, and health management to support successful discharges. Participate in discharge planning, promote safety and quality outcomes, uphold infection prevention standards, and adapt to different units and workflows while delivering patient-centered care.
