Job Summary
Provide on-site patient care using a harm reduction approach while focusing on HIV, Hepatitis C, and chronic condition management. Conduct assessments, diagnoses, and treatment planning to prescribe and manage medications, educating patients on prevention and wellness. Collaborate with physicians and interdisciplinary teams to maintain accurate documentation in EMRs, adhering to CNO standards and privacy regulations. Engage local health and community organizations to deliver compassionate, community-based healthcare across Ontario. Full-time and part-time roles available with relocation support in Thunder Bay.
Required Qualifications
- Registered NP (Adult or Primary Health Care) with active CNO license
- Masters degree (MN-NP or MScN)
- Current N95 Mask Fit Test and immunizations (MMRV, TB test, or chest X-ray)
- Proficient in EMRs
