Job Summary
Perform admission and reassessments for all residents, triaging cases to determine healthcare needs and develop comprehensive plans of care. Provide day-to-day oversight of personal care aides while interacting with primary care and hospital providers to ensure resident requirements are met. Monitor and reevaluate care plans continuously, adhering to all regulatory guidelines and participating in quality assurance activities and chart audits. This role is situated in an assisted living facility in Troy, NY, serving the elderly and mental health populations with a focus on high-quality assessments and compassionate supervision.
Required Qualifications
- Active NYS Registered Nurse License
- 1-3 years experience in an Assisted Living or Long Term care setting
- understanding of and passion for the elderly and/or mental health population
