Registered Nurse - PM Shift - Per Diem

Follow the hospice care plan, provide skilled palliative care, and coordinate services for patients with terminal illnesses. Work cooperatively with the attending physician and interdisciplinary team, participating in care conferences and supervising Home Health Aides. Perform prescribed medical treatments including pain management and symptom control, conduct assessments, and provide education to patients and caregivers. Report changes to the RN case manager and review the plan of care regularly. Requires current CA RN license, BSN preferred, and one-year recent nursing experience. Per diem PM shift from 4:00pm to 12:30am with a $2.00 shift differential.
