Job Summary
Registered Nurse, Phone Triage (on-site) provides high-quality telephone triage, clinical assessment, care coordination, and patient education under Nurse Manager direction. Conducts symptom assessment, determines acuity, directs patients to appropriate level of care, schedules appointments, completes forms, documents in EMR, and supports quality improvement. Requires ADN with NY RN license and at least 3 years of nursing experience in acute/outpatient/ambulatory/primary care; EPIC/EMR proficiency; strong clinical judgment, communication, and teamwork skills; on-site in Rochester, NY.
Required Qualifications
- Graduate of an accredited School of Nursing
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) strongly preferred
- Active and unencumbered New York State RN license
- Minimum of three (3) years of nursing experience in acute care, outpatient, ambulatory, or primary care settings
