Job Summary
Registered Nurse (RN) plans, organizes and directs home care services with emphasis on community health education. Responsibilities include completing initial patient and family assessments, developing and revising care plans, administering medications, providing direct patient care, counseling patients and families, coordinating discharge planning, and serving as Case Manager when assigned. Additional duties include on-call participation, supervising home health aides, preparing clinical notes, and maintaining communication with physicians and community health partners to coordinate care within patients’ homes and communities.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in state
- CPR certification
- Bachelor’s degree, with one year of home health care experience preferred
- 1-2 years of recent acute care experience in an institutional setting
- Ability to lift, position or transfer patients
- Able to lift supplies and equipment
- Valid driver’s license and automobile
- Management experience not required
