Job Summary
The Patient Navigator coordinates and manages care for patients within a defined clinical service program (e.g., total joint replacement, cancer treatment coordination) by working with the multidisciplinary team to educate patients and families, address psychosocial and financial barriers, and ensure seamless transitions across care settings throughout the episode of care. Responsibilities include developing customized care plans, facilitating education materials for specific programs, supporting resource allocation, identifying and addressing care gaps through data-driven evaluation, and contributing to program goals and quality initiatives. Strong collaboration with staff, physicians, and leadership to optimize outcomes and cost-effective care is required. This role also involves advocacy, education, and outreach to patients, staff, and community members, with supportive benefits and opportunities for growth within Monument Health.
Required Qualifications
- Completion of a nursing education program that is approved by a board of nursing
- Basic Life Support (BLS) Certification - American Heart Association
- Registered Nurse (RN) - South Dakota Board of Nursing
- BLS certification within 60 days of hire or transfer (as applicable)
- Preferred: Bachelor's degree in Nursing
- Preferred: 1+ years of Case Management Experience
- Preferred: 1+ years of Patient Care Experience
