Job Summary
Bridge hospital and outpatient care to reduce fragmentation; provide ongoing clinical monitoring and early intervention for post-discharge patients; empower patients through personalized health coaching, education, and support for long-term treatment goals; coordinate care plans among primary care providers, specialists, and community resources; address social determinants of health and medication management; willing to travel within a 40-mile radius for in-home visits and specialty appointments; focus on improving patient outcomes within Total Quality Management and ambulatory settings.
Required Qualifications
- Licensed Registered Nurse obtained prior to hire date
- Registered Nurse credentialed from the Oklahoma Board of Nursing
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
