Job Summary
Proactively identify and welcome new clients, conducting thoughtful assessments to deeply understand their unique needs and ensuring they stay connected to care. Provide informal counseling, health education, and steady social support beyond medical logistics. Help clients navigate the system by scheduling vital appointments, attending visits as a supportive companion, and simplifying the medical application process. Act as the glue connecting the department with vital community partners and referral sources to foster collaborative relationships for holistic health. Maintain Tennessee Board of Nursing licensure or a valid multi-state compact license. Full-time day schedule at Saint Thomas Health in Nashville, TN within the Population Health department.
Required Qualifications
- Licensed Registered Nurse credentialed from the Tennessee Board of Nursing
- current home state license for multi-state license recognition 'Compact State'
- obtained prior to hire date or job transfer date
- Diploma from an accredited school/college of nursing
- Required professional licensure at time of hire
