Job Summary
Assess physical, social, and psychological needs of patients through observation, interviewing, and physical examination to develop care plans. Coordinate multidisciplinary care by managing referrals, ordering supplies, and ensuring delivery to patient homes or facilities. Document all services in accordance with government regulations and VNS Health policies while communicating with team members and management. Transport medical equipment weighing 25–30 lbs to varying environments using approved transportation. For NFP roles, provide health education and social support to pregnant and postpartum mothers and their infants. Maintain a flexible work schedule to meet client needs and participate in required training sessions and special projects.
Required Qualifications
- License and current registration to practice as a registered professional nurse in New York State
- Valid driver's license or NYS Non-Driver photo ID card
- Degree in Nursing from an approved program
- Bachelor's Degree
- Minimum of one year of experience in a medical/surgical environment (for CHHA)
- Minimum of one year of clinical nursing experience, including at least six months of Hospice, or Med/Surg or ICU or Emergency Dep't or Home care, Palliative Care, Oncology or Long Term Care experience (for Hospice)
- Minimum of one year of recent experience in maternal/child health (for NFP)
- Demonstrated basic computer skills
Desired Qualifications
- Willingness to obtain Wound Care Certification within one (1) year of hire (for Hospice Only)
- Certification in Hospice and Palliative Care Nursing (for Hospice Only)
- Nursing required (for NFP Only)
