Assess, diagnose, and implement patient-centered care plans in the Ambulatory Care Clinic, collaborating with physicians and nurse practitioners to achieve expected outcomes. Collect healthcare data systematically, evaluate patient progress, and educate patients and families on health adjustments. Differentiate professional nursing practice from delegated tasks while participating in unit councils, quality improvement programs, and clinical inquiry activities. Maintain awareness of evidence-based practice and current regulations, including the NY State Nurse Practice Act and The Joint Commission standards.
