Job Summary
Complete initial and physical assessments of patients and families to determine home care needs, then observe and report changes in condition to the supervisor. Initiate and revise plans of care based on nursing diagnoses, incorporating therapeutic actions while counseling patients and families. Administer prescribed medications, provide direct patient care, and implement discharge planning as part of the care plan. Act as Case Manager to coordinate patient care for assigned caseloads, prepare clinical notes for providers, and communicate with community health persons. Required Qualifications
- Graduate from an accredited registered nursing program
- Licensed in the state
- At least one (1) year clinical practical nursing experience
- Possess and maintain valid CPR certification
- Must have reliable transportation
- Current driver's license
- Appropriate automobile insurance
Desired Qualifications
- preferably in intensive care, rehabilitation or medical surgical nursing
- Home health experience
