Job Summary
Provide home health nursing care as a Registered Nurse in Lakeland, FL. Responsibilities include conducting comprehensive patient assessments, developing individualized care plans with the care team, reviewing medications and vital signs, coordinating care through referrals and clinical documentation, communicating with the clinical manager and interdisciplinary team, educating patients and caregivers to promote independence, coordinating with physicians and therapists, supervising LPNs to ensure high standards of care, and performing additional duties to support patients and the team. Requires travel to patient homes and the ability to work independently within a home health setting.
Required Qualifications
- Current Registered Nurse (RN) license in the state of Florida
- At least 1 year experience as a home health nurse
- Knowledge of nursing care methods and procedures
- Knowledge of health and safety guidelines and procedures
- Team player with excellent communication and interpersonal skills
- Excellent organizational and multi-tasking skills
- Problem-solving skills
- Familiar with EHR such as Kinnser
- Ability to work independently, manage a daily schedule, and travel between patient homes
