Job Summary
Conduct comprehensive patient assessments, develop individualized care plans, and review medication and vital signs in collaboration with the clinical team. Coordinate care by making referrals or documenting clinical rationale, communicate patient status updates to physicians and therapists, and educate patients and caregivers on self-care techniques. Supervise licensed practical nurses to ensure high standards of care while performing additional duties as needed to support team operations. Requires a current Florida RN license, at least one year of home health experience, familiarity with Kinnser EHR, and ability to manage a daily schedule involving travel between patient homes. Full-time positions include health, dental, and vision insurance, 401(k), paid time off, and holidays.
Required Qualifications
- Current Registered Nurse (RN) license in the state of Florida
- at least 1 year experience as a home health nurse
- Excellent knowledge of nursing care methods and procedures
- In-depth knowledge of health and safety guidelines and procedures
- A team player with excellent communication and interpersonal skills
- Outstanding organizational and multi-tasking skills
- Patient with excellent problem-solving skills
- Familiar with EHR such as Kinnser
- Ability to work independently, manage a daily schedule, and travel between patient homes
- Level 2 background screening in compliance with Florida AHCA regulations
