Job Summary
Deliver compassionate, high-quality direct patient care while managing the patient's comprehensive care journey, meticulously documenting progress, and executing physician and nursing orders promptly. Coordinate patient services across all clinical disciplines as the primary liaison to guarantee seamless care, and facilitate discharge planning by proactively assessing patient needs and collaborating with the team for a safe transition home. Ensure all clinical actions adhere to established, patient-first best practices and protocols. Required Qualifications
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date
- American Heart Association or American Red Cross accepted BLS Provider
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date
- Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire
