Job Summary
RN provides direct patient care and case management in home health, including admissions, developing care plans, and coordinating care with the interdisciplinary team. Responsibilities include obtaining medical history, performing physical assessments (vitals, mobility evaluations), educating patients and families on care plans, delivering skilled nursing care with HIPAA compliance, documenting visit records, updating care plans, coordinating with LPNs and Home Health Aides, and facilitating referrals for services such as therapy or social work. Requires CA RN license, experience with OASIS/WellSky (Kinnser) preferred, med-surg/wound care experience a plus, current physical exam and TB test within specified windows, CPR certification, valid driver’s license, and ability to attend orientation at Yuba City office. The role offers part-time and per diem opportunities with per-visit pay and benefits for full-time employees, serving AZ, CA, CT, FL, IL, IN, MA, MN, NV, OH.
Required Qualifications
- Active RN license in California
- Experience with OASIS and WellSky (Kinnser) strongly preferred
- Med-surg and wound care experience a plus
- Current physical exam (within last 6 months)
- Current TB test (within last 12 months) and CPR certification
- Valid driver’s license
- Must be able to attend orientation at our Yuba City office
