Job Summary
Admissions RN leading client admissions and assessments for an in-home care agency. Conduct in-home assessments and develop personalized care plans; coordinate admissions paperwork and ensure policy compliance; communicate with families, caregivers, and physicians to ensure continuity of care; support clients with clinical expertise during the transition to in-home care. Requires active Virginia RN license and at least 1 year of nursing experience (home health or admissions experience preferred). In-home visits across Orange, VA; part-time or full-time with flexible scheduling.
Required Qualifications
- Active RN license in Virginia
- Minimum 1 year of nursing experience (home health or admissions experience preferred)
- Strong assessment and communication skills
- Reliable transportation and valid driver's license
