Job Summary
Lead the implementation and continuous monitoring of personalized care plans to ensure high-standard treatment tailored to patient recovery goals. Coordinate care across multiple clinical disciplines as the central communication hub, aligning the entire healthcare team toward optimal outcomes. Assess and facilitate the patient journey from admission to discharge, collaborating with the medical team to ensure safe, seamless transitions back to the community. Communicate critical changes in patient condition in real-time, providing precise clinical insights for rapid and accurate intervention.
Required Qualifications
- Licensed Registered Nurse credentialed from the Tennessee Board of Nursing
- current home state license for multi-state license recognition 'Compact State' obtained prior to hire date or job transfer date
- Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date
- Diploma from an accredited school/college of nursing
- Required professional licensure at time of hire
