Registered Nurse, Oncology Nurse Navigator

Provide disease management for oncology patients by monitoring symptoms, addressing treatment side effects, and triaging medical issues in person or by telephone. Collaborate with the interdisciplinary team to coordinate care, develop multidisciplinary plans, and communicate pertinent health issues to patients and families. Document assessments, interventions, and patient education in the electronic medical record while assisting with medication administration, central line care, and procedure support. Manage medication access, precertification, and service utilization to maximize reimbursement and patient satisfaction. Covers for team members to ensure efficient service delivery and resolves variances to achieve positive patient outcomes. Works onsite 8:00a-4:30p, M-F.
