Job Summary
Registered Nurse (RN) On-Call providing in-home post-acute care coverage in Detroit and surrounding Michigan areas. Schedule includes Monday-Friday 4pm-9pm with on-call from 9pm-8am; travel to patients’ homes to deliver skilled nursing care, administer medications, perform OASIS assessments, develop and adjust care plans, supervise nurse aides and LPNs, educate patients and families, maintain Medicare/Federal-compliant documentation, and collaborate with interdisciplinary team. Requires autonomous practice, strong time-management, compassionate communication, and adherence to agency policies. Includes mileage reimbursement, benefits, and potential sign-on bonus; home-based work with occasional in-service trainings and mandatory meetings.
Required Qualifications
- Registered Nurse license in the state of employment
- Current RN license in state of employment
- Minimum one-year experience as an RN in an acute care setting
- Valid driver’s license and auto insurance
- Home workspace designated for chart reviews if needed
