Required Qualifications

• •current Registered Nurse (RN) license in state(s) of practice
• •Graduate of an accredited school of nursing
• •Ability to pass background, consumer report and drug test
• •Valid State Driver's License (if driving)
• •automobile in good working order (if driving)
• •automobile insurance in accordance with state and organizational requirements (if driving)
• •acceptable driving record (if driving)
• •Pushing/Pulling and Lifting/carrying up to 30 pounds
• •Occasionally up to 50 pounds
• •Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting
• •Available to receive and respond to calls from Triage during assigned shifts
• •Completes comprehensive and ongoing assessment of patient and family
• •Provides a complete physical assessment and history of current and previous illnesses
• •Provides professional nursing care by utilizing all elements of the nursing process
• •Assesses and evaluates patient's status
• •Writing and initiating Plan of Care
• •Re-evaluating patient and family/caregiver needs
• •Participating in revising the plan of care as necessary
• •Initiates the plan of care and makes necessary revisions as patient status and needs change
• •Uses health assessment data to determine nursing diagnosis
• •Develops a plan of care that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions
• •Includes the patient and the family in the planning process
• •Initiates appropriate preventive and rehabilitative nursing procedures
• •Administers medications and treatments as prescribed by the physician
• •Counsels the patient and family in meeting nursing and related needs
• •Provides health care instructions to the patient as appropriate per assessment and plan
• •Assists the patient with the activities of daily living
• •Remains with patient and/or family through a crisis period until resolved and/or relieved
• •Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given
• •Records pain/symptom management and changes/outcomes as appropriate
• •Responsible for maintaining compliance with all HIPAA regulations and requirements
• •Treats all patient information as confidential
• •Utilizes protected health information on a minimum necessary basis
• •Works from 5:00 PM to 8:00 AM Monday-Thursday
• •24 hours on weekends Friday-Sunday
• •Rotating schedule of 7 days on and 7 days off
• •Handles a variety of hospice nursing care depending on the current needs of patients and the organization
• •Hospice patient visits
• •Admissions
• •Responding to urgent night and weekend patient needs
• •Filling in for other nursing staff

Desired Qualifications

• •1+ years of recent experience in hospice/home care
• •2+ years of Nursing experience
