Job Summary

Respond to triage calls for hospice patients with changing conditions, completing weekend admissions and after-hours visits. Conduct comprehensive physical assessments, evaluate patient status, and initiate or revise plans of care based on nursing diagnoses. Administer prescribed medications, counsel families, and maintain accurate clinical notes while ensuring HIPAA compliance. Remain available for crisis periods until resolved, adhering to a rotating schedule of 7 days on and 7 days off from 5:00 PM to 8:00 AM Monday-Thursday and 24 hours on weekends. Requires current RN license, 2+ years of nursing experience, and valid driver's license if driving is needed.

Required Qualifications

  • current Registered Nurse (RN) license in state(s) of practice
  • Graduate of an accredited school of nursing
  • Ability to pass background, consumer report and drug test
  • Valid State Driver's License (if driving)
  • automobile in good working order (if driving)
  • automobile insurance in accordance with state and organizational requirements (if driving)
  • acceptable driving record (if driving)
  • Pushing/Pulling and Lifting/carrying up to 30 pounds
  • Occasionally up to 50 pounds
  • Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting
  • Available to receive and respond to calls from Triage during assigned shifts
  • Completes comprehensive and ongoing assessment of patient and family
  • Provides a complete physical assessment and history of current and previous illnesses
  • Provides professional nursing care by utilizing all elements of the nursing process
  • Assesses and evaluates patient's status
  • Writing and initiating Plan of Care
  • Re-evaluating patient and family/caregiver needs
  • Participating in revising the plan of care as necessary
  • Initiates the plan of care and makes necessary revisions as patient status and needs change
  • Uses health assessment data to determine nursing diagnosis
  • Develops a plan of care that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions
  • Includes the patient and the family in the planning process
  • Initiates appropriate preventive and rehabilitative nursing procedures
  • Administers medications and treatments as prescribed by the physician
  • Counsels the patient and family in meeting nursing and related needs
  • Provides health care instructions to the patient as appropriate per assessment and plan
  • Assists the patient with the activities of daily living
  • Remains with patient and/or family through a crisis period until resolved and/or relieved
  • Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given
  • Records pain/symptom management and changes/outcomes as appropriate
  • Responsible for maintaining compliance with all HIPAA regulations and requirements
  • Treats all patient information as confidential
  • Utilizes protected health information on a minimum necessary basis
  • Works from 5:00 PM to 8:00 AM Monday-Thursday
  • 24 hours on weekends Friday-Sunday
  • Rotating schedule of 7 days on and 7 days off

Desired Qualifications
