Job Summary
Coordinate and provide skilled nursing care to individuals and families during on-call hours, completing initial assessments and responding to requests for phone instruction, counsel, or home visits. Deliver direct patient care, manage drug and sharps disposal, and teach caregivers proper medication and equipment use. Provide support at the time of death and complete all charting according to hospice policy. Work across seven counties, potentially visiting inpatient units, while maintaining electronic documentation via iPad and iPhone. Requires a current Kentucky license, two years of RN experience, and the ability to access homes independently.
Required Qualifications
- Graduate of an accredited school of nursing
- Current Kentucky License
- Must be able to work with an interdisciplinary team and be a team player
- Must be able to do electronic documentation with the company provided iPad and iPhone
- Must be able to access patient's homes independently
- Cannot have any lifting restrictions
- Must be available for on-call Monday-Thursday from 5pm-8am
- Must be able to work in the inpatient unit
Desired Qualifications
- Two years experience as a professional nurse preferred
- Minimum one year in acute setting
