Job Summary
Registered Nurse for the Diabetes Education Program at North Lambton Community Health Centre. Provides evidence-based education and diabetes management support for individuals and families with, or at risk for, type 2 diabetes. 1.0 FTE; 37.5 hours/week based at West Lambton/Sarnia with travel to other sites as needed. Interdisciplinary team collaboration; emphasis on education, program development, and community health impact. Benefits include vacation, medical, pension, and paid time off. Location: West Lambton, Sarnia, with potential travel to other sites. Remote option: in-person.
Required Qualifications
- Undergraduate degree in nursing from a recognized university, or combination of nursing diploma and additional relevant courses/training
- Current Registration as a Registered Nurse (RN) with the College of Nurses of Ontario
- Training and experience in diabetes/pre-diabetes education programs and support services. Diabetes Educator Certificate preferred
- Three to five years nursing experience in a community setting or combination of community and hospital or public health settings
- Experience in program development, implementation, monitoring and evaluation
- Proficiency in the use of computers and various software applications
- Strong interpersonal, public speaking and communication skills, including the demonstrated ability to explain complex health concepts to the lay public
- Proficiency in French, is considered an asset
