Job Summary
Conduct initial assessments and admit new members into the AFC/GAFC program, then perform regular home visits to administer nursing care and monitor patient symptoms. Educate patients on health maintenance, provide emotional support to families, and facilitate referrals to specialists while maintaining accurate medical records. Communicate directly with collaborating physicians regarding patient care changes. Requires previous nursing experience with AFC/GAFC program guidelines and strong critical thinking skills.
