Registered Nurse (NE) - Per Visit (PRN)

Provides professional nurse care in home health and hospice for per-visit (PRN) duties. Conducts initial and ongoing patient assessments, develops and updates individualized care plans, administers medications and treatments as prescribed, counsels patients and families, identifies discharge planning needs, and initiates appropriate preventive and rehabilitative nursing actions. Serves as case manager for assigned caseload, coordinates care with primary physicians and community health professionals, prepares clinical notes, and communicates patient status every 60 days as required. Participates in on-call duties and supervises home health aide care every two weeks. Maintains compliance with the State Nurse Practice Act and supports Comfort Hospice Care’s mission of providing compassionate, 24/7 care; offers equal opportunity employment.
