Registered Nurse Navigator

Serve as the central point of contact and care coordinator for patients with suspected or diagnosed concussion or mild traumatic brain injury. Guide patients and families through the continuum of care from initial referral through recovery, ensuring timely access and evidence-based, patient-centered treatment. Coordinate care including triage, scheduling, follow-up visits, imaging, neuropsychological testing, and specialty referrals while performing nurse-level assessments under established protocols. Engage with contracted school districts to facilitate communication, provide parent education, and support professional development. Collaborate with physicians, therapists, and case managers in interdisciplinary team meetings to review complex cases and update care plans. Document all encounters in the electronic health record and contribute to refining clinical pathways and patient education materials.
