Job Summary
Consult patients by phone or in person to determine specific needs and referrals, while building a patient-family-centered relationship. Provide initial admission assessments, screenings, and barrier identification, then develop and deliver evidence-based care plans with treatment summaries. Attend provider appointments to reinforce education, manage consents and declinations, and follow up with both patients and providers as required. Maintain a sustained patient-centered relationship throughout the course of care and into post-care. Requires two years of direct patient and provider experience, a Colorado RN license, BLS certification, and an Associate Nursing degree. Hourly pay ranges from $40.56 to $64.68.
Required Qualifications
- 2 years of experience working directly with patients and providers
- Minimum of Associate Nursing Degree
- Licensed as a Registered Nurse in the state of Colorado or from a valid compact state
- Basic Life Support (BLS)
- Registered Nurse Certified – Inpatient Obstetric Nursing (RNC-OB)
Desired Qualifications
- 2 years of experience in specialty associated with application
- Oncology Certified Nurse (OCN)
