Job Summary
A professional registered nurse (RN) who offers individualized assistance to patients, families and caregivers to help overcome health care system barriers. Navigates the patient through the continuum of care by coordinating, triaging, educating, and serving as the point of contact for patients and families. Collaborates with physicians and interdisciplinary teams to manage patient care, facilitates shared decision making, and enhances patient engagement and health literacy to align treatment decisions with patients’ values. Works to improve outcomes and sustain navigation activities within the clinical program or service line, supporting positive outcomes for patients, providers, and the Sutter Health system.
Required Qualifications
- Education: Graduate of an accredited school of nursing
- Certification & Licensure: RN-Registered Nurse of California
- BLS Healthcare Provider
- Typical Experience: 5 years recent relevant experience
- Knowledge of medical terminology
- Verbal and written communication skills
- Microsoft Office software experience required, including Excel and Word
- Ability to ensure follow through with responsibilities and communicate with a varied level of health care staff
- Knowledge of third-party reimbursement helpful
