Job Summary
Assess patient needs and develop individualized plans of care for acute care patients in hospital, observation, or virtual settings. Administer direct patient care, including medication, treatments, and equipment use, while documenting progress and ensuring safety. Collaborate with interdisciplinary teams and medical staff to coordinate care, delegate tasks appropriately, and support the hospital at home program by traveling to patient homes and using telehealth technology. Serve as a preceptor, charge nurse, and unit educator to mentor new staff and students. Maintain compliance with all policies, attend staff development, and participate in peer review and performance improvement committees. Requires current Alabama RN license, BLS/ACLS/NRP/PALS certifications, and a valid driver's license.
Required Qualifications
- Graduate of an approved school of nursing
- Current State of Alabama RN license or valid eNLC multistate RN license
- Current BLS/Healthcare Provider certification
- ACLS, NRP, PALS, TNCC (required for certain areas)
- NRP (required for Neonatal Intensive Care Unit)
- Valid driver's license
- Reliable transportation to travel to perform job duties for Hospital Care at Home In-home nursing
- 1 year experience
Desired Qualifications
- Experience as a preceptor, charge nurse, unit educator, and/or nurse clinician
- Knowledge of principles of growth and development over the life span
- Competency in nursing skills and use of patient care/unit equipment as defined by unit/department-specific requisite skills
- Ability to prioritize patient care in an ongoing manner in accordance with Evidence-Based Practice Standards of Care
- Effective problem identification and resolution
- Competency to work autonomously and be productive in a hospital at home environment
- Capability to use in-home virtual technology to connect with the Patient Care Hub and conduct virtual visits for patients in the home
- Ability to travel and enter patient homes as the patient hospital at home and place of service within a specific geographical region
- Capability to use TeleHealth technology to conduct virtual patient care visits to our hospital at home patients
- Assessment of clinical symptoms and ability to escalate potential patient decompensation
- Competency to address any urgent and/or emergent patient needs as directed
- Transportation and delivery of items, equipment, and medications (excluding controlled substances and IV chemotherapy drugs) as requested
