Registered Nurse

Supervise and support Certified Nursing Assistants under the direction of the Director of Nursing while collaborating with the interdisciplinary team to assess, plan, implement, and evaluate individualized care plans. Ensure initial baseline and periodic comprehensive assessments are completed on time, prepare for physician rounds by flagging concerns and documenting reviews, and initiate consultations or referrals based on resident needs. Deliver nursing care within scope of practice, monitor residents for changes in condition such as confusion or mobility loss, and notify providers as needed. Perform administrative tasks including medical documentation, evaluations, and reports. Active RN license required; experience in skilled nursing or long-term care preferred.
