Job Summary
Registered Nurse (RN) – PACT role provides overall medical care in the community setting, including basic nursing assessments, client/family education, diagnostic procedures, physician consultations, and execution of physician orders; 75% of care delivered in the community. Requires RN license in Virginia (or multi-state) and accredited nursing program completion; BSN preferred. Responsibilities include nursing assessments, patient education, coordination with an interdisciplinary team, medication administration, documentation, and travel to Horizon sites as needed. Emphasis on community mental health and public health knowledge, with opportunities for ongoing education and loan repayment programs. Full-time position with possible travel and flexible schedules as required.
Required Qualifications
- Graduate of an Accredited nursing program
- Licensed Registered Nurse in the state of Virginia or eligible for appropriate multi-state license
- BSN preferred
- One year of nursing experience as an RN required
- Ability to drive and travel to Horizon worksite or other locations as needed
