Job Summary
MDS Nurse-led role responsible for organizing, completing, and evaluating resident assessments and care plans in accordance with state and federal guidelines; coordinates MDS, CAAS, and care planning upon admission; conducts monthly reviews, maintains ARDs, ensures nursing documentation reflects care plans, engages with residents/families, collaborates with interdisciplinary teams, participates in Quality Improvement/Quality Assurance, supports training on MDS documentation and electronic health records, and provides resident-centric care while promoting dignity and well-being.
Required Qualifications
- Current unrestricted license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in practicing state
- A nursing degree from an accredited college or university
- 2+ years of MDS experience, skilled nursing experience preferred
