Coordinate the MDS, CAAS, and care planning processes for each resident upon admission; complete and evaluate resident assessments to ensure highest possible level of functioning; establish ARDs and care plan dates, maintain accurate nursing documentation, and report changes to the interdisciplinary team; provide training on required supportive documentation and EHRs; mentor MDS nurses to foster professional growth; participate in Quality Improvement/Quality Assurance programs and respond to inspections; demonstrate proactive resident advocacy and collaboration with physicians and families.
