Job Summary
Coordinate and deliver comprehensive nursing care in collaboration with interdisciplinary teams to achieve optimal patient outcomes. Perform patient assessments, triage, and ongoing monitoring to develop individualized care plans. Administer medications, immunizations, IV infusions, and wound care while recognizing changes in patient condition and responding to medical emergencies. Educate patients and families on health promotion and treatment plans. Coordinate referrals, specialty appointments, and follow-up care to ensure continuity. Supervise nursing personnel and maintain accurate documentation using electronic health systems. Assist providers during examinations and procedures. Manage electronic patient messages for Family Medicine teams, including reviewing messages and preparing prescription renewals and referral requests. Hands-on clinical duties are limited to occasional injections or assisting physicians.
Required Qualifications
- Baccalaureate Degree in Nursing, a Graduate Diploma in Nursing, or an Associate Degree in Nursing from an accredited agency such as the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
- Must be in person through the American heart Association or American Red Cross
- Basic Life Support (BLS)
- Advance Cardiac Life Support (ACLS)
- Pediatric Advanced Life Support (PALS)
- One year of experience in nursing
- Successfully completed at least 12 classroom hours of continuing education within the preceding 24 months that maintains skills and knowledge as an Registered Nurse
- Possess and maintain a current unrestricted license to practice as a Registered Nurse in any one (1) of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands
- Provide two (2) letters of recommendation from practicing physicians or peers attesting to the your clinical skills
- Reference letters must include name, title, phone number, date of reference, address and signature of the individual providing reference
- Letters of reference must have been written within the preceding two (2) years
