Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent hospitalizations. Coordinate care across providers, complete timely documentation, and adjust plans based on interdisciplinary team communication. Work full-time Monday through Friday with a shared on-call rotation for Southern Jackson and Northern Monroe County communities.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license
